Navneet K. Sharma MD

Navneet Sharma, MD is an established and highly regarded physician and medical director and is President and CEO of Tri-County Health and Wellness. Dr. Sharma is Board Certified and has been practicing as a physician for the past 16 years in the greater Philadelphia area. He has worked as a Hospitalist since his start as a physician, in 2004, and continues to practice as a hospitalist physician today while remaining a Medical Director/Vice Chair/Independent Affiliate for a number of Medical Organizations throughout the region. Dr. Sharma is an experienced physician leader and entrepreneur with vast experience and strong business development while ensuring patient satisfaction and the utmost respect for patient care throughout the community.

Noah Bass DO

Dr. Bass has over 20 years experience providing compassionate Primary Care, Acute Hospital Care and individualized patient-centered Addiction treatment. Dr. Bass is board-certified in Family Medicine. He received his medical degree from Lake Erie College of Osteopathic Medicine and completed Residency training at St. Luke's Hospital in Allentown, PA.
Dr. Bass is passionate about Patient education. Dr. Bass believes in empowering Patients to confront addiction and preventative healthcare in a supportive, non-judgemental environment. Dr. Bass received Top Doctor Awards in 2018 and 2019.

Nicole Oesterling is a Nurse Practitioner at Tri County Health & Wellness Center specializing in adult primary care and aesthetic medicine. She attended West Chester University obtaining a Bachelor of Science degree in nursing graduating with honors. After working for several years as a Registered Nurse, she continued to pursue her dream of becoming a Nurse Practitioner. She graduated from Neumann University in 2016 obtaining a Master of Science Degree in nursing, graduating with honors. Nicole is a board certified Adult Gerontology Primary Care Nurse Practitioner.
Nicole grew up local to the area graduating from Owen J. Roberts High School. She began her career in 2001 as a Registered Nurse at Phoenixville Hospital working in multiple aspects of healthcare including, medical/surgical/progressive care, emergency department, critical care and nursing leadership/administration. She worked at Phoenixville Hospital serving the community for fifteen years. After obtaining her Nurse Practitioner board certification from the American Nurses Credentialing Center in 2016, she began working for Tri County Hospitalists at the Pottstown Hospital focusing on hospital/internal medicine. Her sense of leadership, motivation and dedication to the profession advanced her to a Lead Nurse Practitioner role within her organization and she was involved in several prestigious elected committees.
Nicole is a member of several professional organizations including the American Nurses Credentialing Center (ANCC), American Nurses Association (ANA) and The ChesMont Nurse Practitioners Association.
Nicole is passionate about health and wellness and is dedicated to provide high quality and compassionate care to exceed her patients’ expectations. Nicole will be seeing patients in the primary care office and she is the director of Pure Bliss Medical Spa, both specialties are located in the Tri County Health and Wellness Center.
Nicole is married with three children. She enjoys spending time with her family at their vacation home at the Chesapeake Bay. Nicole is her children’s’ #1 fan and loves spectating at their sporting events. She also cheers for the Philadelphia Eagles and 76ers!
Alexandra Baltazar Wallace, PA-C
Physician Assistant
Alex is a board-certified Physician Assistant. She attended Connecticut College and received her bachelor’s degree in Behavioral Neuroscience. After graduating, she continued her education at the University of Bridgeport, receiving her Master of Science in Physician Assistant Studies. She is passionate about providing quality and compassionate care to patients of all ages.
Born and raised in New England, she moved to Chester County in the Summer of 2023 with her husband. In her free time, she can be found at her local CrossFit gym, skiing, and spending time with her family.

Ashley Dahl PA-C
Physician Assistant
Ashley is a board-certified Physician Assistant. She earned her Bachelors of Science in Respiratory Care from Texas State University in San Marcos, Texas. She later pursued a Master of Science in Physician Assistant Studies at Philadelphia College of Osteopathic Medicine. She is committed to offering outstanding, compassionate, and thoughtful care to every patient she serves.
She was born in San Antonio, Texas and grew up throughout various states/countries due to her family being in the military. Her favorite place was living in Germany due to the food and experiencing the different cultures. In her free time, she enjoys exploring new coffee shops, spending time outdoors hiking, and traveling.
